Title: Multisite Replication Study of the Transitional Care Model

Move the Needle is a Nursing Study that focusing on the TCM model.
The Transitional Care Model (TCM) is an advanced practice registered nurse (APRN) led, team-based, care management strategy designed to improve the care and outcomes of high-risk older adults transitioning from hospital to home.
Despite the fact that use of the TCM has been advocated by scholars and policy makers [20-22], uptake of this model by health systems has been slow and time intensive.
This study is administered by VREF and funded by Arnold ventures, University of Pennsylvania School of Nursing, and NIH.
